Introduction
Living Well Taking Control (LWTC) is a subsidiary of the charity Health Exchange. (LWTC) is a non-profit organisation that collaborates with the NHS to deliver the Healthier You Programme and Local Authorities to deliver Weight Management Programmes. Our mission is rooted in the belief that proactive intervention and lifestyle modifications can significantly reduce the risk of developing Type 2 Diabetes, ultimately leading to healthier, happier lives.

The Role:
LWTC is looking for an experienced Health Coach based in the North Northamptonshire area to deliver two health programmes on a part time equivalent basis. The successful candidate will lead on the delivery of the LWTC Weight Management Programme in North Northamptonshire and support the wider LWTC Team in delivery of the NHS Healthier You Behavioural Support for Obesity Prescribing Programme.
In this role, you will lead engaging group sessions, motivate participants, and adapt delivery to suit cultural and community needs — all within the programme’s established standards and specifications. You will create a safe, inclusive, and supportive environment, using behaviour-change tools such as SMARTE goal setting, COM-B, problem-solving, and setback planning to ensure every participant is guided throughout their journey.
This is a highly autonomous role: you have control over your schedule and choice of local venues, while ensuring every session meets LWTC and NHS guidelines. You will also act as an ambassador for the programme, promoting its value, maintaining confident and accurate delivery of LWTC curriculum, and helping service users stay engaged, informed, and motivated from their first assessment through to the final session.

LWTC Weight Management Programme
The 12-week weight management programme by Living Well Taking Control (LWTC) is a free, supportive, and practical programme designed to help individuals develop sustainable healthy eating and activity habits. Through expert guidance, nutritional advice, and lifestyle coaching, participants can achieve long-term weight loss and improved well-being. The LWTC Weight Management Programme is the primary programme delivered within the role.
NHS Healthier You Behavioural Support for Obesity Prescribing Programme
The 9-month national programme is an NHS commissioned programme designed to provide Comprehensive, practical, and person-centred support for patients on weight loss journeys alongside pharmacological treatment.

- Knowledge of weight management, nutrition and physical activity in relation to health improvement
- Able to use communication skills to motivate and encourage patients to follow appropriate behaviour change and treatment programmes, using empathy, reassurance and analysis to uncover social/psychological factors that may affect a patient’s ability to make positive changes, to achieve long-term lifestyle changes.

Living Well Taking Control – What do we supply
Living Well Taking Control Specific Training to be able to deliver the LWTC Weight Management Programme and Healthier You Programme to Full Capacity.
Locality Coaches are provided with fully funded IT equipment – laptops and mobile phone.
Support from a dedicated regional Compliance & GP Liaison Officer who will help with monthly activity, assist in connection with referral sources and set up primary care networks access across your locality.
Provision of central marketing and media support to help deliver locality-based Website, Social Media campaigns and printed documents such as leaflets.
Where does this happen
Locality Coaches will be responsible for delivering the LWTC Weight Management Programme in person in North Northamptonshire and Healthier You Programme remotely. The LWTC Weight Management Programme is the primary programme delivered within the role.
Financial Aspect (what do you earn)
You are in the driving seat and determine how much you earn.
Locality Coaches (LCs) once signed off and authorised will be eligible to commence direct intervention sessions and accept interventions up to a maximum cap of 550 interventions per annum (FTE).
An advanced payment scheme is in operation once the number of interventions per annum is agreed, payment will be made for the number of interventions pro rata. In addition to the advanced payment LCs will invoice a fee per patient on completion of each of the 4 milestones within each programme. The fee per patient will be programme specific. Target for each milestone and conditions for claim will be detailed in advance of delivery
LCs can negotiate additional referrals above the cap of 550 (FTE) but these will be paid at PBR rates only.
Potential to earn Up to £35,000 or £40,000 for Full Time Equivalent – depending on the agreed targets

- Proof of clear Enhanced DBS
- 2 x References
- Public Liability Insurance
- Satisfactory completion of Living Well, Taking Control service specific training
ELIGIBLITY TO WORK IN THE UK
All Locality Coaches must be eligible to work in the UK. We only accept applications from candidates who are legally entitled to work in the UK.

All Locality Coaches are required to present an Enhanced Disclosure and Barring Service (DBS) check

Locality coaches must provide proof of certification in the areas listed below. If qualified coaches are finding access to these courses a challenge, LWTC can facilitate access to free or low cost courses.
